As we age, the skin of our upper eyelids begins to droop and rest on the upper eyelashes. For some patients, there is a family history and the process can begin in our forties.
If the skin is heavy and excessive, it can obstruct vision and surgery can become medically necessary. The incision for upper eyelid surgery is made in the upper eyelid crease, and is well hidden when healed.
Bags related to lower eyelids can be a genetic feature, or part of the aging process. Bags are caused by loosening of the orbital septum, and prolapse of the fat around the eyes.
Depending on the depth of the eye socket, the surgeon may not achieve the same result in all patients having lower eyelid surgery. The incision for lower eyelid surgery is made just under the eyelash line and is inconspicuous when healed.
A festoon is a puffy swelling on the cheek, just below the lower eyelid. As it is in the skin of the cheek, not the lower eyelids, it has to be treated separately.
Laser lipolysis, which involves using a laser to liquefy fat and induce skin contraction, is ideal for mild cases of festoons. Severe festoons may have to be excised surgically if the patient wishes to have them treated.
Browlift surgery may be done towards the temple, or in the centre of the forehead or both. In young females, the outer corners of the eyebrows are often higher than the inner corners of the eyebrows; this gives an 'open’ appearance to the eyelids and is re-created in a temporal browlift.
Male brows are heavier and more horizontal, so male browlifts have to take that into account. Overdoing browlifts is to be avoided as it can give a 'surprised’ look that is difficult to reverse.
Medicine and natural preparations that increase the risk of bleeding need to be stopped a week before planned eyelid surgery. Stopping smoking will help the healing process and also slow the ageing process.
Discomfort is mild after eyelid surgery, and sleeping with the head elevated on a pillow for a few days helps to decrease swelling. Bruising subsides in about two weeks and strenuous activity should be avoided during that time to minimise the chance of bleeding.
The eyes will be sensitive to light initially so sunglasses are essential for the first week after surgery.
An anti-wrinkle treatment such as Botulinum Toxin is still most effective for forehead frown lines and the deep grooves at the glabella as those lines are the result of muscle activity.
Once the muscle activity is controlled, dermal fillers such as Aquamid or Restylane can help to fill the furrows for a smoother look.
Anti-wrinkle treatment can also help the crows feet and smile lines at the corners of the eyes. However, there is no treatment for dark circles around the eyes, apart from camouflage make-up.
