Liposuction involves the removal of large amounts of fat from the body. Liposculpture is a procedure that is suitable when smaller amounts of fat need to be removed from the body.

Liposuction

Surgical liposculpture is achieved using the technique of liposuction, during this surgery hollow tubular instruments of various sizes are used to extract body fat. Tiny incisions are made at the site where fat is to be removed, and a solution is infused to provide anaesthesia, reduce bleeding and improve fat extraction.

During liposuction a hollow cannula is inserted into the area via the tiny incisions. The cannula is then attached to suction tubing, through which the fat is aspirated.

Knowing how much to leave behind is a guide to how much fat should be removed, and this is a joint decision between the patient and the surgeon; and what is achievable based on the surgeon’s aesthetic judgement and experience.

Liposuction - Suitability

It is important to have realistic expectations of the results of liposuction. Older patients who have skin that has lost its elasticity are more prone to rippling, dimpling and loose skin after liposuction.

Liposuction may not be the most suitable procedure for patients with excess folds of skin, which is best removed by a tummy tuck, arm reduction or breast lift/ reduction.

Liposuction does not improve muscle tone either. If a patient gains weight after liposuction, the procedure may need to be repeated. If the patient loses weight, excess skin may need to be trimmed in a subsequent procedure.

Liposuction and Recovery

The recovery phase of liposuction takes several weeks. In the first week after liposuction, the patient will experience bruising and ooze from the incisions.

It is important to wear the compression garment for several weeks to help shape the areas that have been treated by liposuction. The garment also helps to control swelling, which settles slowly over several weeks after the surgery.

The discomfort from liposuction is moderate and usually settles in the first fortnight with the help of simple analgesia. Overall, liposuction is a useful surgical technique and has a niche in plastic surgery practice.

Injection Lipolysis

An alternative to liposuction surgery is chemical liposculpture or injection lipolysis involves the "off-label" use of phosphatidylcholine or lecithin.

It was found that injecting phosphatidylcholine in the fatty tissue dissolves fat over several weeks. The treated areas are painful and swollen for a few days after the injection.

The treated areas can also be uneven, as the volume of fat dissolved is not directly related to the volume of solution injected.

Unlike liposuction, the result is not easy to control because of this unpredictability of response of the fatty area injected. However, it may have a role for the patient who wants some reduction in the volume of fatty tissue, does not want to undergo liposuction and is not particularly fussy about the final shape achieved.
